Physical Disability Care

Our Physical Disability Care provides specialised support and assistance to individuals living with physical disabilities. These services aim to help individuals with physical limitations maintain their independence, enhance their quality of life, and overcome daily challenges by receiving care and support within the comfort of their own homes.

Here are key aspects of ourPhysical Disability Care:

Dedicated Personal Care Assistant(s) and mobility support:

We provide support for individuals who have difficulty with mobility, including transferring between surfaces (e.g., bed to wheelchair), using assistive devices (e.g., walkers, wheelchairs), and practicing safe transfers.

Personal Care:

We help with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and mobility.

Nutrition Support:

We assist with meal preparation and ensuring that individuals receive proper nutrition and hydration.

Communication:

We provide communication support for individuals with speech difficulties due to their physical disability.

Medication Management:

We assist with medication administration, ensuring the individual takes their prescribed medications correctly.

Care co-ordination:

We will work closely with other Health Professionals involve in the individual’s care to ensure that all aspects of care are well-coordinated and in line with the individual’s goals and preferences.

Physical Therapy:

We will collaborate with other Health Professionals such as physiotherapists to implement prescribed exercises and rehabilitation routines aimed at improving or maintaining muscle strength, joint flexibility, and overall physical function (if possible).

Occupational Therapy:

We will work with Occupational Therapists to develop strategies and adaptive techniques to enhance the individual’s ability to perform daily activities independently (if possible).

Pain Management:

We will administer any prescribed medication (oral and topical only) to manage pain associated with their physical disability through medication, physical therapy, and other pain management techniques.

Catheter and Ostomy Care:

We provide care and support for individuals with catheters or ostomy bags, ensuring proper hygiene and maintenance.

Home Safety Assessment:

We regularly evaluate the home environment of the individual to identify potential hazards and making necessary modifications to ensure safety.

Emotional Support:

We offer emotional support and companionship to address the emotional challenges that may accompany physical disabilities.

Emotional Support:

We work with individuals to set rehabilitation goals and provide ongoing support to achieve them.

Our Physical Disability Care Package is highly individualised, with care plans tailored to the specific needs and goals of each individual. Our primary aim is to empower individuals with physical disabilities to live as independently as possible while receiving the necessary assistance and support to maintain their physical and emotional well-being. Our Personal Care Assistants are trained in physical disability care ensure that individuals receive compassionate, person-centred care within the comfort of their own homes.
