Palliative Care

Our Palliative Care services are specialised healthcare services provided to individuals with serious, life-limiting illnesses in the comfort of their own homes. Our goal is to improve the quality of life for the individual facing a terminal illness by managing symptoms, addressing emotional and psychological needs, and supporting them and their families through the end-of-life journey. Our Palliative Care package allows individuals to receive this comprehensive care within their familiar and supportive home environment.

Here are key aspects of our Palliative Care Services:

 Dedicated Palliative Specialist Personal Care Assistant(s)

We will provide trained dedicated Specialist Personal Care Assistant(s) to provide care and support round the clock.

  Personalised Palliative Care Plan and Advance Care Planning:     

We will provide a Person-Centred Palliative Care Plan including the development advance directives, living wills and other documents that outline the individual’s healthcare preferences.

 Nutritional Support:

We address dietary needs and preferences, ensuring that the individual receives appropriate nutrition to maintain their strength and comfort.

Coordination with other Community Based Health Professionals:

We will work closely with other health professionals involved in the individual’s care to ensure that all aspects of care are well-coordinated and in line with the individual care plan and preferences.

Pain and Symptom Management:

We will work closely with other health professionals involved to focus on alleviating pain and managing symptoms such as nausea, shortness of breath, fatigue, and anxiety, with the aim of enhancing comfort and quality of life.

Medication Management:    

We help with administering and managing medications to control symptoms and maintain comfort.

 Emotional and Psychological Support:

We provide emotional support and counselling to individual and their families, helping them cope with the emotional challenges of a serious illness.

Hospice Care Transition:

If appropriate, we will work with other health professionals to ensure a seamless transition into hospice care when the individual’s condition reaches the end stages.

 Spiritual and Cultural Care:

We address the spiritual and cultural needs and beliefs of the individual and family members, providing spiritual care as needed.

Bereavement Support:

We can offer grief counselling and support to family members after the individual passing.

Our aim is to provide compassionate, holistic care that respects the individual’s wishes, values, and goals. Our Care plans are personalised, and the focus is on enhancing the individual’s comfort and dignity during the end-of-life journey. Our home-based Palliative Care allows individuals to remain in a familiar and supportive environment, surrounded by their loved ones, as they receive specialized palliative care services.
