Our support is therefore designed to offer a personalised care solution to those wishing to maintain and/or improve their independence to ensure every service user receives a tailored service that meets their specific requirements

Who we help 

Chelmscare is registered and regulated by the Care Quality Commission and provides a wide range of Specialist Community Care and Support Services to adults (18-65) and older adults (65 over) with:

Learning Disabilities or Autistic Disorder – (LD & AD)

Physical Disabilities (including Multiple Sclerosis) – (PD)

Mental Health Needs (Such as mild Cognitive Impairment, Anxiety and Depression) – (MH)

Dementia and Memory Loss – (DML)

Parkinson – (P)

Epilepsy – (EP)

Mild Challenging Behaviour (MCB)

Chelmscare is engaged in the provision of a wide spectrum of personalised Specialist Care and Support services to individuals in the comfort of their own homes.

All our Personal Care Assistants has been carefully chosen for their naturally caring attitude and have received all the required trainings to ensure that are able to meet all the assessed needs of those who uses our service.

Our flexible Person and Community Centred Care and positive risk approach promote independence, well-being, integration, recovery, and rehabilitation. All our Care Services are provided in a manner where the cared person is in full control of the type and level of care / support that suits their lifestyle.

Our ethos and philosophy

Working with each service user, social workers, relatives, Personal Care Assistants (as appropriate) and all others associated with his/her care towards identifying, implementing and reviewing a Personal Support Plan which adequately reflects his/her needs at any one time
Promoting independent living, working within the service user’s range of abilities and competencies
Delivering care in the service user’s home in a manner, which is non-discriminatory, sensitive to the cultural needs of the service user and respectful of their environment and traditions
Providing a high-quality flexible, responsive and non-intrusive service that is tailored to the needs of the individual
Ensuring that confidential information is protected at all times and only shared with others strictly in accordance with its policy on confidentiality

Working in partnership with other agencies involved in care in order to ensure a seamless and cost-effective service
