Dementia Specialist Care

Our Dementia Specialist Care provide specialist care for individuals living with dementia in the comfort of their own homes. Dementia is a progressive neurological condition that affects cognitive functions, including memory, reasoning, and the ability to perform daily tasks. Our Dementia Specialist Personal Care Assistants are trained to support individuals with dementia and their families, focusing on maintaining safety, quality of life, and dignity throughout the progression of the disease.

Here are key aspects of our Dementia Specialist Care Package:

Dedicated Dementia Specialist Personal Care Assistants:

Our Dementia Specialist Personal Care Assistants have received specialist training in Advance Dementia Care, including understanding the different types of Dementia (for e.g. Alzheimer’s disease, vascular dementia), their stages and the unique challenges they present.

Personalised Dementia Care Plan:  

Our Dementia Care Plan are personalised to each individual’s needs, preference, and routines, with a focus on maintaining the individuals identify and dignity.

Personal Care:

We provide assistance with personal care tasks, such as bathing, grooming, and dressing, is provided to enhance the individual’s comfort and dignity.

Nutrition and Hydration:

We ensure that individuals receive proper nutrition and hydration, addressing any shallowing difficulties or dietary restrictions.

Medication Management:

We assist with medication management, ensuring individuals take their prescribed medications correctly and monitoring for side effects.

Cognitive Support:

We engage individuals in cognitive exercises, activities, and reminiscence therapy to stimulate mental function and enhance memory and communication skills.

Care co-ordination:

We will work closely with other Health Professional involved in the individual’s care to ensure that all aspects of care are well-coordinated and in line with the individual’s goals and preferences.

Behavioural Management:

We will implement strategies to manage challenging behaviours often associated with dementia, such as aggression, agitation, and wandering.

Daily Living Assistance:

We provide support with activities of daily living (ADLs) such as bathing, dressing, grooming, and meal preparation, while encouraging as much independence as possible.

Communication Support:

We use effective communication techniques to engage with individuals with dementia, including validation therapy and redirection.

Companionship and Social Engagement:

We encourage social interaction and participation in meaningful activities to reduce feelings of isolation and depression.

Safety Measures:

We implement safety precautions within the home environment to prevent accidents and reduce the risk of falls and injuries.

End-of-List Care:

We assist with transitioning to end-of-life care when appropriate, providing comfort, dignity, and support during the final stages of dementia.

Our Dementia Specialist Care is designed to create a safe and supportive environment for individuals with dementia while allowing them to remain in the familiarity of their own homes.We work collaboratively with other healthcare professionals to develop and adjust care plans as the disease progresses .Our goal is to enhance the quality of life for individuals with dementia ,promote their well-being, and support their families throughout their caregiving journey.
